Formulary or pharmacopoeia by Badr al-Dīn Muḥammad ibn Bahrām ibn Muḥammad al-Qalānisī al-Samarqandī (بدر الدين محمد بن بهرام بن محمد القلانسي السمرقندي, fl. 1194) divided into forty-nine chapters (باب). The chapters are listed with their titles in the preface (f. 2r-2v).

The copy was completed on 1 Jumādá II 988/14 July 1580 by ‘Abd Allāh ibn Maulānā ‘Arab Kaḥḥāl (عبد الله ابن مولانا عرب كحال, see colophon Section at the end of a manuscript text. , f. 110v, lines 13-14, transcribed below).

Begins (fol. 1v, lines 2-3):

الحمد لله الحكيم القاهر العزيز القادر المتفرد بالأزلية المتوحد بالأبدية الخير بالسرائر البصير بما في الضمائر

الذي أخرجنا من العدم إلى الوجود بفضله وضرب علينا سرادق طوله ونصب لنا معيار عدله ...

Ends (f. 110v, lines 8-11):

... شعر الدب إذا أخذ من ذنبه شعر ووضع على الأذن

وشرب الإنسان النبيذ لم يسكر ما دام الشعر على أذنه قال عبد الله ابن جبرائيل

في منتخباته من طبائع الحيوانات وخواصها قد جربت هذا فصح و

يقلل السكر جدًا والله أعلم
